Add/Drop Form

This is the Add/Drop form to be used by undergraduate students. Any course changes will not be official until the form is signed by your course instructor and academic advisor, and then signed by the Registrar's Office. You must hand-deliver your completed form to the Registrar's Office.

*If you are unable to get in-person signatures of your instructor or advisor you may secure email permission instead, but you must present printed copies of the emails when you turn in your add/drop form to the Registrar's Office.
