The Master's Program Form should be completed at Pre-Registration for the final semester and handed in before your final semester of classes begin.
When you have completed 15 hours of course work, you must consult your advisor to make sure you will meet all of the graduate course level requirements for graduation. The form is completed and signed by the beginning of your last semester.
This program must have the approval and signature of the chairperson/graduate director of your major department. The form must then be submitted to the Graduate Program Office and approved by the Registrar. The information may be typed or clearly written.

After your form is filled out, please print and gather the appropriate signatures on the new hard copy.
